Prior to your first visiting with Living Hope Clinic, our staff would kindly like to direct you to download a form based on your individual needs.

Insurance
Living Hope Clinic accepts many insurance plans. Please arrive to your visit with your insurance card and a photo ID. We accept all major credit cards as well as checks and cash (exact amount only, as we do not offer change).

Medicare and Medicaid
Both, Medicaid and Medicare are accepted at Living Hope Clinic. All Medicaid services are handled through Optum Management Service. If you would like to request more information, please consult your therapist.

Employee Assistance Programs
Living Hope also accepts Employee Assistance Programs that offer Mental Health coverage. Upon your visit, please be prepared with an authorization number from your program, and be aware of the number of sessions your program will authorize for your needs. Patients are typically authorized between 6-8 sessions by such such programs.
